This happen to me too.

I try to open "options" when I checkout a Maven proyect, on build, a messeges appear ask me for setting in options the maven path or something. Then on the Output windos show me a Link to options, I'm click it and nothing happen. either in tools-> options. Even if a reset the windows.

Finally I know what happened. The window appear in a deactivated monitor. I use a laptop and am usually extend my desktop with another monitor. And naturally some times only work  with the laptop screen.  Then the window is restored under coordinates out side of my current view port.  

This behavior happen with all windows. Even with the main window. It must be fixed with a easy validation.
