I imagine this has been filed, but...
During a debugging session, I've applied code changes (Hot swapped)
and as a result, I've removed some breakpoints (using the editor).  So they 
don't show up in the document, but they still break.  The show up in the 
breakpoint viewer, so I remove them from there.  Is something in NB out of 
sync or is this predicted behavior?

You are right -- fix & continue is not perfect in current implementation (see
issue #57457). We plan to change the whole approach (see also #58450);
unfortunately it will not be trivial.

Verified ... and Closing all issues resolved into NetBeans 6.7 and earlier.
