Issue Summary:
--------------
Annotations with the same name are shown many times in new options dialog, e.g.
current program counter. We need to display each of the annotations only once.

Steps to Reproduce:
-------------------
1. Open new OD.
2. Go to Fonts & Colors | Annotations.
-> Notice that some annotations are displayed more than once.
