If debugger hit breakpoint in a file not open, debugger open source file in a new tab but the breakpoint is not visible and cannot be disabled.

What language do you debug? Does this occur in Java debugger?
Can you please check if the bug persists in NetBeans 8.1?

Java debugger was affected, but this is likely a general problem of not correctly drawn annotations in editor, which was fixed in 8.1.
