1) Create  a form
2) Rename it via explorer
3) Expand the form node
4) Look at its class subnode and form node .

Consequently when you would open the form then its Form Window would have the original name in its caption .

Maybe already fixed. Pretty old bug report...
Probably relates to bugs 4224 and 6679 (which also cannot be fully reproduced).
