Operating System = Linux version 2.6.31-20-generic running on amd64
Java; VM; Vendor = 1.6.0_15
Runtime = Java HotSpot(TM) 64-Bit Server VM 14.1-b02

To reproduce:
Right click on a folder in Files window 
In context menu move mouse over the New  menu item
The third and the seventh item in the New context menu are the same: PHP class

Cannot reproduce because templates are ordered using LRU (Least Recently Used) algorithm - it means that my entries are completly different than yours :)

If you know how to reproduce, feel free to reopen and provide the steps.
