- Bring up Tools>Options>Keymap; sort by Actions; scroll to "Close Window" there are two bindings for it, Ctrl+W and the alternative Ctrl+F4 - click "Close Window (alternative shortcut)"'s "..." and select "Add Alternative" Observe that an new/empty alternative is added, but it's shortcut is not selected for editing. The shortcut for the original alternative is in edit mode. Click on the newly added empty alternative and it disapears.

My apologies, Ernie. It indeed is reproducible. I should have paid more attention to your steps, the key one was to 'sort by Action'. Sorry.
