On step 5, there is no way to make the Next or Finish button enabled (build 
206).

I cannot reproduce this behaviour.

To enable Next button you should select one existing menu item (not only 
e.g. "Tools") - to get placement for new menu item.

Maybe it's not clear and it should be commented on Step 5 page.

I descrease the priority because it's only bad commented behaviour

Fixed. I changed text on the top of panel to:

Add menu and toolbar items. Select existing menu item to get placement for the 
new one.
