The IDE should hint to change the file extension to PHP if there seems to be some PHP syntax in it. For example when the user starts typing PHP script 
opening tag.

When changing the file extension it would be highly desirable to automatically update all the references to it (see issue 123545).

PHP file can have html (or any other) extension if the server allows it. It is not desireable to support this.
