When diagrams are dirty, exiting ide or closing dirty uml project comes up with the the Save dialog that only shows the
icons for diagram but no associated diagram names. see attached screenshot.

Repro steps:
- create uml project of any type
- create a diagram of any type
- add a couple of elements to the diagram
- close the uml project or exit the ide to see the Save dialog

Furthermore, this happens only for diagrams created during that NB session. Diagrams that existed prior to the current
NB session don't have this problem.

Diagram name is now included in Save dialog for new, dirty diagrams.
There was also a problem with existing diagram that when opened, then renamed, on close, the save dialog shows the old
name. This has been an issue since back to 5.5.1. 
All has been fixed.
