When answering a new quiz, if you have already submitted an answer, there is only the Submit button and the checkbox for
email notification at the bottom of the page.  I think you should still put in the Name, Email, Country and (optional)
University fields, pre-filled with the previous values, so the user is not confused about who is logged in and who is
submitting the answers to the current quiz.
