The "Refresh for entity under cursor" action on the Hierarchy pane display the hierarchy of the opened file in the editor instead of the selected file on the project list (if this file isn't opened).

As designed.
There are two action "Hierarchy" and "FIle Hierarchy". Hierarchy uses context for resolving the type which should be inspected. The File Hierarchy does not use the context and inspects a class in selected file. So use File Hierarchy (Ctrl+F12 on Mac).
