Actions in the tools menu are not uninstalled when a module is disabled

To reproduce:
-Install a module which installs an action to tools menu (i.e. Servlet module).
-Disable it in global settings
-The actions (i.e. create JSP for a database table node) will still be installed
-Enable it again
-The actions will be present twice.
