1. start NB 8.1
2. install ODCS plugin
3. add server and login
4. Team > Report Tasks...

I understand these should be replaced with numbers.

those sections should not be shown for a new, not yet submitted, issue

can be reproduced only when menu > team > report task... is invoked

the issue dialog isn't properly initialized when it is opened - this was addressed in issue #256040
and is marked as a candidate for the next 8.1 patch release
