NOTE: This issue is platform-specific and appears to only 
occur on Windows NT.

If the user attempts to replay a request for a servlet in 
a context other than the default context, it will not be 
replayed properly and no new record will be recorded by 
Monitor.

This works properly for JSP requests and servlet requests 
in the default context.

This seems to have been caused by bad proxy settings in 
Internet Explorer.  I adjusted the proxy settings and it 
is working fine.
