Ubuntu 11.10 Live CD x64
JDK 1.7.0_01 x64

1. File | New Project
2. Select Java EE | EJB Module, Click Next
3. Click Next
4. Change Java EE Version to J2EE 1.4

Checkbox and Label is shown
Set Source Level to 1.4.

This old bug may not be relevant anymore. If you can still reproduce it in 8.2 development builds please reopen this issue.
