The designation of "Charset" isn't contained in the generated HTML
file from "Generate Documentation".
Because of this, when an HTML file is indicated, misconversion
is sometimes caused. 

To reproduce: 
	- At Filesystems tab in Explorer Window, right click the XML file
		and select [Generate Documentation].

Added a <meta> tag generation.
Generated content is always explicitly written as UTF-8 encoded
instead of platform default.
