There are missing icons by "Debug", "Set As Main Project" and "Run SQL" in the "Customize Toolbar" dialog (right-click
the toolbar, choose "Customize Toolbar"). If you place these actions to the toolbar, some universal icons ("file with a
question mark") are assigned to them.

It seems, that "Run SQL" icon is missing only in the customize toolbars dialog. It's displayed correctly after d'n'd to
toolbar panel.
