1) Invoke Text File wizard
2) Type a name
3) Go to 3rd page "Choose Extension"

ERROR A:
Panel is higher than the previous panels (e.g.
"Name and Location")
ERROR B:
There is missing step description in the left
column. Only step 1 (Choose File Type) & 2 (Name
and Location) are visible.
ERROR C:
When you type new extension, the "Add to Supported
Extensions" checkbox is still disabled.

BTW: Why the 3rd page can't be part of 2nd page in
the bottom part? (See e.g. Web Module Listener wizard)

"Text File" wizard has been probably replaced by "Empty file" wizard
in 200406101800 build.

Is it as designed?

Correct. Text module was removed and it's functionality was absorbed
by core modules.
